En attendant la version numérique, voici une proposition de bulletin d'inscription pour impression.
En version odt et pdf

Remarques :
je me suis inspiré du bulletin d'adhésion de franciliens.net

A vérifier :
Il y a une demande de carte d'identité, mais je ne suis pas sur que ce soit nécessaire. (a vérifier)

Question :
comment gère-t-on la signature avec la version numérique ?

La carte d'identité nous permet de vérifier l'âge. Car pour les mineurs, il faut une lettre d'un responsable légale.
J'ai modifier le document dans ce sens.
